That said, I ought to give your suggestion serious consideration.
Doesn't it breach ImageShack's T&C? I can't imagine they'd like someone
using their bandwidth without users getting to see their advertising...

Interesting. So they prefer you to use thumbnail links which lead to
their quite intrusive advertising (graphic links surrounding the target
image & pop ups), but they do allow hot linking directly to uploaded
image content so one can avoid the ads.
I'm still a little suspicious as I don't understand their business model
& why they aren't being exploited. Are you aware of any
problems/short-comings/catches?
There's only one point I'm not clear on.
T&C 2 Terms of Service includes: "Changing ImageShack® linking codes is
prohibited without prior permission."
Linking codes include (using a specific image by way of example):

I take T&C 2 above to imply one can't change the above or use linking
code not in their list.
What I'm not clear on is if the direct link (# above) to an image can be
used on a website or rather merely used within, say, personal emails,
news posts & so on with the Hotlink code (* above) required for website
use (thus supplying imageshack with some non-intrusive advertising, not
unreasonably, whenever you embed the image (see $ below)).
At first I thought their FAQ implied # could be used on a website:

However now I'm more inclined to think they're using 'direct linking' in

($) Why am I asking this (aside from me being anal)?
Simply as some pre existing Gallery software won't allow images to be
presented as links, which (*) requires.
Also, if one is concerned with accessibility issues, (*) above abuses
the use of the alt attribute & would reduce accessibility.
Any ideas?

I would not bother with fancy gallery software. I would just throw the
thumbnails onto a home-made webpage and line them up nicely using tables
or styles.

What I would do, to keep ImageShack happy, is use the "thumbnail code"
"for sites", for example:

I would also add a target="_blank" to the link, so that the link will
open in a new browser window. That way whoever's browsing the gallery
will keep Imageshack happy, but not always have to press their browser's
Back button... they can just close the tab or window.
That way I'm happy, ImageShack is happy, and the site's visitors are happy.

I'm not keen on removing control from the user. I'd let them decide
whether to open a link in a new window or not (two clicks in Firefox
rather than one - not much work).

Their restriction on not changing link code is qualified by "without
permission". I offered to link each gallery page to their site &
prominently credit them for the image hosting; they replied I could
alter the code as I liked :)
All in all they seem to offer a very useful service. I still don't know
how they make money out of it, but I shan't complain :)

Ad revenue from all the silly people who don't know how to make their
own links and so dutifully use the provided ones.
Believe me, you can get filthy rich off of ad revenue alone. Take Eric
Bauman for example. He's a pathetic little man who built a "fun" site
full of plagiarized content, and now he has a collection of Ferraris.
