Working on a consumer product

Hi all,

I'm currently working on a consumer product and was wondering if there is
somewhere that I can get a hold of a soft-copy (hard if absolutely
necessary) of a Veterinary Patient Chart.  What I mean is the form that a
Vet fills out when you take your pet in for a visit.

......Now if you want to make a really useful consumer product come up with
a standard form for vets to fill out, answering questions about their
practice philosophy, including schooling and CEU credits.

Perhaps it's none of my business; but, if
you are thinking about making a practice
management system or software, be
informed that there are several dozen
already on the market and a few "majors"
that are affiliated with chains of animal
hospitals, veterinary drug suppliers,
laboratories etc.
