9 yrs old doberman, suddenly showed symptoms of sluggishness and one night
wanted to urinate repeatedly and was very uncomfortable. Following this
was put on a course of cifran. 3-4 days later blood test showed low
Hemoglobin and high blood urea and creatinine. The first impresion was
kidney damage and was given IV gencef for 4 days this was followeby a oral
course of cifran 250 mg for 4 days. One injection erythropoietin (human
recombinanat) has been given. The next blood report showed normal blood
urea and creatinine, but low Hb and platlets and high TLC count. Post this
the dog wa on a course of microdox DT- for 9 days. The next blood report
shows hemolytic anemia. The hemoglobin has fallen further and TLC and
Neutrophils are still high. Today she ahs been stated on Monocef IV. IV
dextrose has been given on every second day. The urine report shows loss
of albumin and RBC in the urine.
There is complete loss of appetitie, refusal of food and water, no willing
ness to move and lack of response, shivering .

Suggest abdominal ultrasound to rule out lesions on any organs. One
of the greyhounds placed by my group recently had symptoms similar to
this; he had histiocytic sarcoma.

I don't know what most of those drugs are, am assuming antibiotics of some
sort. Has an xray or ultrasound of the abdomen been done? I'd be very
suspicious of some sort of cancer, like hemangiosarcoma. You could also be
dealing with immune-mediated hemolytic anemia, would need to run tests like
coomb's, ana, rf. Don't know where you are, but around here I'd also be
suspicious of tick-borne disease, and would treat with doxycycline.

Abdomen Ultrasound done- it shows that there is a encapsulated mass - 75x
75 mm on the right lobe of the liver, with complex echo. It indicates a
mass or an abscsis. Not very sure.
The left kidney shows hydronephrosis and calculi.
there is mylena as she is loosing blood in the stool.
She has been on a cource of doxycyclin orally for 9 days. Now she is on
cephradine 500 mg intravenous
the dog is in india and there is no provision to do a coombs test or
protozoan tests.

Sounds like you may be dealing with cancer. You'd need to biopsy the liver
mass to know for sure. You can look for autoagglutination as an alternative
to a Coombs test -- you mix a drop of blood with a drop of saline -- if it
clumps then you know you have some sort of immune-mediated disease. Outside
of doing that, I'd probably be giving some hefty doses of prednisone along
with the antibiotics to see what kind of response you got. Sounds like a
sick puppy and a tough case. Good luck.
