What could've caused this?

Dane pup, 9 weeks old, 29 lbs. went in for his crop.  He had a full blood
profile, heart check and physical pre-op, all results normal.

He went under and was on heart monitor.  Cropping was done.  No problems at
all.  They removed him from the anesthesia machine (?) and placed him on
oxygen, and he stopped breathing.  They tried to revive him for 20 minutes,
but failed.  A necropsy is being conducted, but the results won't be back
for 2 weeks.  There are others who've said that they've had similar stuff
happen, and the necropsy didn't show anything.

Here is what was used on him:

Atropine Pre-Anesth 60-less
Valium injection
Torbugesic injectable
Telazole 0.01
Isoflurane Anesthesia 1/2 hour
Torbugesic injectable(again)

OK, just generally speaking, (as I understand you might not want to speak to
specifics of this event) what can cause a young dog to die on the table like
this?  Since this dog is a puppy is it possible that there was some
congenital problem which didn't show up in the bloodwork?  And Suja mentions
a dog that died on the table and the necropsy showed nothing?  How is this
possible?
